Medical coverage is needed by all
You can get medical coverage through Relias, your spouse’s or domestic partner’s employer, or another source. However, being covered is required under the health care reform law, so Relias strives to provide you with the best plan and price for you and your family.
Our plan provides you with prescription drug coverage as well as virtual visits. With virtual visits, you can see a board-certified doctor anytime, anywhere—no wait, no appointment needed. Physicians are available 24/7 through your mobile device or computer to help you with minor health issues such as cold and flu, allergies, and common infections. Finally, we can all use a personal assistant from time to time, especially as we’re trying to navigate the often-challenging world of health care. That’s where your Personal Health Advocate (PHA) through our Health Advocacy Program comes in. Your PHA can help you resolve a wide range of health care and insurance-related issues, and even serve as an intermediary between you and your health care providers and insurance companies.
Note: The benefits you select as a new hire or during Open Enrollment are effective through the end of the calendar year. Your only other opportunity to make changes to your benefits is when you have a qualifying life event.

Finding a Network Provider:
- Go to www.anthem.com and click “Find a Doctor”.
- Search by plan by choosing your state and plan/network. The National PPO (Blue Card PPO) is the network of doctors for our plan.
- Then, enter your provider’s name, specialty or condition, and zip code.
- Click “Search.”
Or, you can contact Anthem at 855-702-1127 for assistance.
The Transparency in Coverage Rules require certain group health plans to disclose on a public website information regarding in-network provider rates and historical out-of-network allowed amounts and billed charges for covered items and services in two separate machine-readable files (MRFs). The following link leads to these MRFs: www.anthem.com/machine-readable-file/search/. The link will allow you to search for your files using your Employer Identification Number (EIN) 95-2949493.

The Relias medical plan offers you a Health Savings Account (HSA) to contribute pre-tax dollars that helps you pay your out-of-pocket healthcare cost, save on taxes, and save for the future.
