Q: How do I access my benefit information?
A: To view your current enrollments in UKG, click on Myself > Benefits > Manage my Benefits > Current Benefits
Q: How do I access my medical, dental, and vision insurance card information?
A: You can access your ID card information by going to the appropriate provider website (Anthem BCBS, Delta Dental, or UHC Vision.) Our vision provider, UHC, does not provide physical insurance ID cards. You may print your information from their website, www.myuhcvision.com, or give your eye care provider your SSN.
Q: How do I update my dependent/beneficiary information?
A: To update dependent/beneficiary information, log into UKG and click Myself > Benefits > Manage my Benefits > My Family. If you need to update enrollments for your dependents, please refer to the “Making Changes to Your Benefits Section”.
Q: What changes can I make to my benefits if I have or adopt a baby?
A: You have 31 days from the qualifying life event (QLE) to add/make changes to your benefits. The qualifying life event is the date of birth of your child, or the date you first get custodianship/guardianship of your child. You can enroll your child, spouse, other dependents in medical, dental, vision, etc. You may also enroll in the Dependent Care Flexible Spending Account and Dependent Life Insurance. When you add a dependent onto your medical, dental, or vision insurance, you will be required to provide information that proves this is an eligible dependent. For a child, you will be asked for his/her birth certificate.
Q: What is a Qualifying Life Event?
A: A Qualifying Life Event allows you to add a dependent to your benefits. Such events are as follows: an employee becomes benefits eligible, marriage, legal separation, birth, adoption, spouse/dependent becomes benefits eligible, etc. To update your benefits, log into UKG and click Myself » Benefits » Manage My Benefits to begin processing your life event. Documentation must be provided within 31 days of the life event. Coverage will retro back to the date of the QLE or the date the documentation is received dependent upon the type of QLE. Q: How do I check my HSA balance?
A: You can check your HSA balance at https://www.netbenefits.com. Funds from the most recent payroll can take up to 6 business days to post to your account. Employer contributions are funded by the end of the applicable month.
Q: How do I change my HSA contribution amount?
A: You can change your HSA contribution amount at any time. To do so, please log into UKG and click Myself » Benefits » Manage My Benefits. You will click Update My Benefits and select HSA Change as your Life Event.
Q: How do I download the Mobile Health app?
A: You can download “Mobile Health Consumer” through the AppStore or Google Play. Once you have downloaded the app, you will click Register Now and will need to provide your First Name, Last Name, SSN, and Date of Birth. All To Health With It Initiatives are available through the app. Alternatively, you may also access the site on your computer at www.mobilehealthconsumer.com.
Q: How do I change my 401(k) contribution percentage?
A: You can change your 401(k) contributions at any time. To do so, log into your account at www.netbenefits.com (first-time users will need to create a username and password). Once logged in: Click Menu » Retirement Savings » Contributions » Contribution Amount. Adjust your contribution amount to the appropriate percent, then click “Change Contribution Amount” at the bottom to submit the change.
